I cannot believe content publishers such as businessweek and Techcrunh are still using media kits. I once called a big name publisher as instructed via their media kit and got a voicemail. Needless to say my online ad budget of $5000 went to facebook without placing a call to anyone. I was ready to buy and they were ready to sell

I found this company: http://www.formmailhosting.com that does the form-to-email part.  Plus they store the results in a MS Excel file which is a bonus.
